Older adult care coordination is offered for residents of Peoria, Fulton, Marshall, Stark, Tazewell and Woodford Counties. In this program, an RN works with individuals to develop a comprehensive care plan that connects older adults with appropriate community-based programs. The RN also does a follow-up check, gives the senior's family periodic updates and regularly reviews the client's needs, revising the care program if necessary.
